Surrendering Your Pet

We understand the decision to surrender your pet is not an easy one and you might be considering surrendering for a variety of reasons. FVHA believes in caring for pets and their people and we would love to get to know you and your pet and figure out how we can best help you. We may be able to offer assistance so you can keep your pet in home or help you rehome your pet directly to a new home.

If you are interested in using the services of FVHA to rehome your pet, we ask that you fill out the surrender intake questionnaire below providing information on your pet’s behaviors and lifestyle questions. This is to aid us in getting to know your pet in order to best set them up for success and find them the right home. After completing the questionnaire, a member of our team will contact you in 48-72 hours. If you feel immediate action needs to be taken, please complete this survey and then call us at 920-733-1717, ext 100 to request assistance.  

FVHA practices a managed admission model and we intake all owner surrenders by appointment only. This insures we have adequate space and resources to ensure that your pet is getting the care they need while they are at our shelter.
